How Do Doctors Decide Which IVF Protocol Is Right for You?
SHARE

In vitro fertilization (IVF) combines eggs and sperm in a laboratory dish, and fertility teams call this plan your IVF protocol. After fertilization, an embryologist monitors the embryos for several days before a doctor transfers one into the uterus. The process follows a set sequence, but the medication plan varies from patient to patient. This is how doctors decide if IVF is the right approach:

Failed Conservative Methods

Many patients try simpler treatments before they move to IVF. Typical options include oral ovulation drugs, and some patients also complete intrauterine insemination (IUI) cycles. Records from these attempts give your doctor useful response data. Your doctor reviews details such as:

  • How many follicles developed on each medication
  • Which doses caused side effects or overstimulation
  • How many cycles you completed without pregnancy
  • Whether ovulation occurred on schedule

Understanding how your body responded to past treatments helps create a personalized plan for IVF. Some patients may have experienced mixed results with prior medications, but others may find that their body responded consistently to specific protocols. By examining these treatment patterns, your doctor can identify what adjustments may lead to better outcomes.

Comprehensive Evaluation

Before your doctor chooses a protocol, they review your full medical history. This review covers past pregnancies, surgeries, menstrual patterns, and current medications. Some details point toward a gentle approach, and others suggest stronger stimulation. Your doctor also asks about any previous fertility treatment.

Blood tests give your doctor a detailed picture of your ovarian reserve and overall hormonal health. These results reveal how many eggs you likely have remaining and how well your ovaries are functioning. If your results suggest a lower reserve, your doctor may adjust your medication doses or recommend a gentler protocol. If your results indicate a higher reserve, your doctor will take extra care to reduce the risk of ovarian hyperstimulation syndrome (OHSS) when selecting your protocol.

A transvaginal ultrasound counts the small follicles in each ovary, and doctors call this number the antral follicle count. The scan also checks for cysts, fibroids, or polyps that might affect treatment. Your age, weight, and conditions like polycystic ovary syndrome (PCOS) also factor into the choice. Each factor narrows the list of suitable protocols.

Male Factor Infertility

Infertility involves both partners, so the male partner also completes testing. A semen analysis measures sperm count, movement, and shape. If results fall below normal ranges, the lab may repeat the test after several weeks. Some men also see a urologist for further evaluation.

When sperm numbers or motility are very low, your doctor will likely recommend intracytoplasmic sperm injection (ICSI). In ICSI, an embryologist injects a single sperm directly into each mature egg. This step replaces standard mixing in a lab dish. Male factor results rarely change your stimulation medications, but they do shape the fertilization method.

Joint Decision

Your doctor presents protocol options based on your test results. Each option has a different medication schedule, and each carries its own risks. You have a say in the final plan. Before you agree to a protocol, ask your doctor these questions:

  • Which protocol do you recommend, and why?
  • How many injections will I need each day?
  • What side effects are common with this plan?
  • What is the total cost, including medications?

Book an IVF Consultation

Doctors choose IVF protocols using test results, treatment history, and your input. No single protocol works for every patient. If you want to learn which approach fits your situation, schedule a consultation with our fertility team. Call an OB/GYN office today, or use an online form to set an appointment.
